What they do

The digital and transformation lead supports access to care through technology and new initiatives, enhancing primary care effectiveness and community service sustainability. They create strategic plans to optimise systems and digital infrastructure, focusing on projects like digital triage, online signposting, social media, integrated NHS services, and promoting the NHS App.

  • Collaborate with surgeries to identify challenges and solutions for effective service delivery.
  • Plan and prioritize changes to enhance care delivery and improve patient and staff experiences.
  • Develop and manage a comprehensive transformation plans for the PCN.
  • Support the effective use of existing and new technology systems for improvement.
  • Advise on innovative opportunities and share best practices to maximise benefits.
  • Develop mechanisms to assess the impact of changes.
  • Provide feedback to PCN leadership and member practices on initiative progress, capacity, and issues.
