Health coaches

What they do

Health coaches assist patients in reaching fitness and lifestyle goals. They promote physical activity, healthier choices, and long-term behavioural changes. After an initial assessment, coaches help patients set personal goals and action plans, motivating them to complete the programs and take control of their own health.

Key aims

  • Setting realistic and attainable goals, while providing the resources and confidence patients need to effectively manage their health and well-being.
  • Offering personalised advice on physical activity and healthy living (e.g. nutrition, smoking cessation, alcohol reduction) to help patients better manage and mitigate the impact of chronic conditions.
  • Encouraging patients with education and easy-to-follow plans that integrate seamlessly into their daily lives.
  • Introducing preventative measures to enhance physical and mental well-being, reducing the risk of long-term conditions.

How to connect with them

Patients will be identified and referred by healthcare colleagues within the surgery. They will visit a health coach just as they would see their GP or practice nurse, typically in a private room. Seeing a health coach does not prevent patients from seeing a GP if needed.
