Pharmacists

What they do

Pharmacists collaborate within the general practice team to enhance the effectiveness and outcomes of medication use. They directly engage with patients, offering additional support for managing chronic conditions, guidance for those taking multiple medications, and increased accessibility to health assessments. This role is essential for enhancing care quality and safeguarding patient well-being.

Key aims

  • Prescribing medications.
  • Conducting comprehensive medication reviews.
  • Addressing patient queries about medications.
  • Providing remote consultations and support.
  • Treating minor illnesses.
  • Ensuring medication adherence and efficacy.
  • Collaborating with healthcare teams for coordinated care.

How to connect with them

Patients can schedule appointments to consult with a pharmacist via their surgery reception team. They will meet the clinical pharmacist in a private room, similar to their consultations with GPs or practice nurses. Patients who see a clinical pharmacist always retain the option to see a GP if necessary.
