What they do
Social prescribers (also known as social prescribing link workers) help individuals improve their well-being by connecting them with local community resources. They provide non-medical support to address social, emotional, and practical challenges that may affect health, such as loneliness, financial stress, or lifestyle changes. Working alongside GPs, they offer guidance, referrals, and practical solutions tailored to each person’s needs.
Key aims
- Offer a non-judgmental, supportive listening ear and provide tools to manage everyday stresses.
- Refer individuals to specialist services for financial advice, housing needs, or domestic abuse support.
- Encourage and support healthier lifestyle choices, including diet, physical activity, and substance reduction.
- Assist in managing long-term health conditions or new diagnoses.
- Facilitate access to educational courses and learning opportunities in the community.
- Connect individuals with mental health support, including counselling services.
- Help combat isolation and loneliness by identifying local social groups and activities.
- Support access to specific health-related support groups.
- Assist with practical issues such as obtaining food, fuel, and prescriptions.

To speak with a social prescriber, ask a member of your GP surgery or healthcare professional for a referral. This service is provided in partnership with Citizens Advice Wirral.
