Social prescribers (also called social prescribing link workers) help people improve their well-being by connecting them with local, non-medical community resources and working alongside GPs to provide tailored, practical support.
They offer a non-judgemental listening ear, guidance, and referrals to services for issues like loneliness, financial or housing problems, and domestic abuse. They also link people to counselling and mental health support, educational courses, social activities and condition-specific groups, and help with healthier lifestyle choices (diet, activity, reducing substances) and practical needs such as food, fuel, and prescriptions.
