Overview
A new initiative from NHS England has launched to develop neighbourhoods. These are local partnerships that bring together health services, social care, voluntary, and community organisations to support people within a defined local area. Teams work collaboratively to better understand the needs of their community and coordinate care and support, with a focus on prevention, well-being, and helping people live independently for longer. By working in this way, services are more joined up, easier to navigate and more responsive to local needs.
Below are some of the voluntary organisations we are working with to improve and support our patients.
Age UK Wirral
Age UK Wirral exists to promote the well-being of older people on the Wirral, helping to make later life a fulfilling and enjoyable experience. You can meet new friends, have fun, and enjoy activities and services.
You can view a list of activities on the Age UK Wirral website and see what services Age UK Wirral can offer you.
